The pharmacy doesn’t effectively identify and manage all the risks associated with its services. It doesn’t have all the procedures it needs to make sure its team works safely. And the procedures it does have aren’t always being followed.

• Implement effective robust, SOPS signed and reviewed, with each staff member aware of responsibilities.
• Carry out a risk assessment, and have risk avoiding measures in place, such a near miss log to review and learn from potential errors to ensure patient safety is adhered to.
• Review medicinal waste handling, and controlled drugs handling according to guidance and standards.

The pharmacy doesn’t adequately review the quality or the safety of the services it delivers.

• Ensure the pharmacy is maintained to high level of cleanliness, ensure when delivering services necessary questions and crucial aspects are consulted with patients and staff

The pharmacy doesn’t do enough to make sure it keeps adequate records for some of its clinical services or in the way the law requires it to do so. These records include its consultation records, controlled drug register, emergency supply records, private prescription records and its responsible pharmacist log.

• Create record keeping folders, record every aspect of any private prescription or services provided to patients/service users so easy to access and refer to it necessary.
• Ensure all staff are aware that RP log must be signed in and out and operation of pharmacy is not allowed to continue without this being addressed.

The pharmacy doesn’t always have enough team members to deliver its services safely and effectively. The pharmacist sometimes struggles to do all the things they need to do. And they rely upon locum staff to help or support them.

• Conduct interviews and employ more staff.
• Ensure the credentials of the staff have been evaluated or set up training courses for the relevant staff.

The pharmacy can’t always show that all its services are provided safely and effectively. And members of the pharmacy team don’t always follow the correct procedures all the time.
The pharmacy doesn’t keep adequate records to show its working practices are safe and effective. And it can’t show it has delivered the right medicine to the right person or that it assembles its compliance packs in a safe way.

• When locum staff members are working, ensure regular staff members including responsible pharmacist delegate responsibilities and duties effectively and efficiently. So their intervention is less likely required and staff understand the way in which pharmacy operate.

The pharmacy can't show it suitably stores all its medicines that it needs to keep in a refrigerator or that it carries out checks to make sure its medicines are safe and fit for purpose.

• Carry out safety check on storage facilities and speak to company if new appliances are required.
• Keep a fridge log connect to the PMR software used Proscript

The pharmacy can't show its team takes appropriate action when the Medicines and Healthcare products Regulatory Agency (MHRA) issues a drug alert. And it isn't always receiving all the drug alerts it should.

• Review MHRA regulatory more effectively, even though we do keep an eye on alerts sent by email, ensure I am receiving all emails print out alerts and notifications and store, them and ensure staff members are aware of these alerts.
