The pharmacy does not identify and assess all the key risks from its activities and services. It does not have a process to complete written risk assessments and audits to help it manage these risks. This includes for the treatments it supplies, unlicensed products and receiving prescriptions from a third party company. The pharmacy misses opportunities to record interventions, which could help identify unmanaged risks.

Risk Assessments

1. Categories
Risk assessment templates created and evaluated for current categories of products sold, e.g. Dermal Fillers, Toxins etc…
2. Unlicensed Products
Template already in place and used for unlicensed products. However, this needs to be reviewed when new products are added to the pharmacy
3. Third Party
Documentation of conversations had with the third party company.
This will include a checklist of activities the third party take to ensure safe practice.
This will be reviewed every 2 years or when there is significant change/a serious incident.
If a new third party is engaged, the same process will be carried out.

The pharmacy does not always follow its own procedures and make interventions when prescriptions contain incomplete information and directions, this includes for weight loss treatments. And it does not verify whether prescribers have face-to-face consultations with patients, when the information on prescriptions is ambiguous.

Training of colleagues
All the pharmacy team, including locums, to be aware of documentation and audits and how to use them. Locums will be informed at the start of their shift.

Review of Interventions SOP in-line with actions

Documenting Interventions
Interventions are currently added to notes on the PMR. However, this could not be easily evidenced. An intervention log has been created for the pharmacy team to log the date, patient details and brief description. This will enable the team to refer to the PMR for the full intervention notes.

BMI
Discussions with IT to add mandatory BMI field for POM weight-loss treatments. There is already a note to request this on the template. If it is not added to the prescriptions, the prescriber will be contacted to gather the information

Audits
Prescription templates currently have a mandatory field where prescribers input the date of the face-to-face consultation. An audit template has been produced to contact prescribers to verify face-to-face consultations have been carried out.
