The pharmacy does not adequately keep or manage the records it needs to help provide its services safely and effectively. This includes records of controlled drugs, records associated with services, and to help achieve proper medicines management.

Record all CD returns in the register that are in the cupboard, and any new ones immediately on receival
Create and provide physical records of any analysis of near miss patterns, risk assessments (such as of equipment for
vaccinations) and complaints/feedback
All staff to be trained on how to correctly use the electronic CD register to be able to alter balances when finding discrepancies
Actioned and witnessed by
Saghir Ahmed on 31st October 23. Some of the staff still need to be trained on the e-CD register as they have not been at work since the inspection but this will be completed within days.

The pharmacy does not effectively manage or store some of its medicines safely.

Organise and tidy all shelves, particularly fast movers, ensuring all
look alike, sound alike’ lines are separated
Ensure all blank boxes or bottles containing medicines have stickers with details of the drug, including batch numbers and expiry dates
Add drug descriptions to all blister backing sheets for all drugs inside
Create and provide physical records of regular date checking
Actioned and witnessed by Saghir Ahmed on 31st October 23. However I
have requested some changes in some bays of medicines are arranged, and some further changes with LASA meds.
