Standard not met | Reason | Action being taken by the Pharmacy | By when | Notification By Pharmacy Improvements Made |
---|---|---|---|---|
1.1 | The pharmacy does not have written procedures for all of its services. And some of the procedures do not reflect current practice. So members of the team may not fully understand what their responsibilities are. |
Review current SOP’s – update accordingly. |

1.2 | The pharmacy does not have written risk assessments for the private services it provides. So it cannot show how it is managing risks to provide its services safely. Members of the team record things that go wrong so they can learn from them. But they do not review the records, so they may miss some opportunities to improve. |
Written RA will be added to private service. Staff do review the dispensing error logs; Superintendent Pharmacist will review error logs for trends on a quarterly basis for trends and discuss with team. |

4.2 | The pharmacy supplies medicines against electronic prescriptions which do not meet the requirements of an advanced electronic signature, and therefore are not valid. Members of the team supply aesthetic medicines without having enough information to be able to provide assurance that they are being prescribed and used appropriately. |
Review current guidance for electronic signatures, and implement via development team. Ask developers to add an “indication” box to the prescription template. Ask developers to add a tick box “I confirm a face-to-face consultation has taken place” to the prescription template. Ask developers to add a box to collect proof of insurance as part of the registration process – discuss how to add this to existing customers. Ask developers to add a date space and name space for staff to add their name and date when the approved a new customer. (Since speaking to the developer this info is captured in the back end already, but will make it viewable) |

4.3 | The pharmacy uses special packaging to protect cold chain aesthetic medicines during delivery. But it cannot demonstrate whether the packaging is effective. So it cannot provide assurance that the medicines will be in good condition when they are received. |
Data log dummy deliveries to ensure ambient and cold chain is maintained – I have received our data logger back after re-calibration and will be doing again but I have old reports when I had a WDL as was doing them then, so I am aware of this. |
