The pharmacy does not adequately identify or manage the risks associated with providing its services. It has consistently demonstrated poor governance arrangements in relation to the management of controlled drugs (CDs). The dispensary is poorly organised, and medicines are not stored appropriately. And it does not make sure all parts of the premises are kept in a clean and tidy condition.

Regular CD checks are to be instigated as per the company SOPs. This will ensure any discrepancies are rectified in a timely manner.

The use of Near Miss Logs and subsequent learning from them to be discussed amongst the team.

Visit from a member of the management team will ensure that the team work together to re-organise the dispensary and ensure that all medicines are stored correctly and shelves are clean.

The pharmacy does not keep all the records it needs to correctly. This includes records about CDs and private prescriptions.

Further training to be carried out to ensure that the branch team are recording CDs correctly and are aware of all legal requirements for Private scripts.
Area Support Manager will be made aware that regular follow-up checks need to be carried out to ensure that this is being done consistently

The pharmacy team is not able to manage its workload effectively, including tasks such as keeping the premises tidy and record keeping.

A new staff member is being recruited who will work on the counter in the mornings giving the foundation pharmacist more time in the dispensary and to enable her to have some study time during the working day.
It will also mean that there will be available time for keeping the store tidy.

The dispensary is poorly organised and cluttered. It has limited clear space to be able to dispense and check medicines safely. There are items on the dispensary floor which are tripping hazards for team members. And the consultation room is untidy. All this is impacting the overall efficiency of the dispensing process. And it may increase the risk of dispensing mistakes or accidents.

There should be adequate work space in the dispensary for the volume of scripts being processed as long as the team keep things tidy.
A visit from a member of the management team will be arranged so that together they can ensure the floor is cleared and the work surfaces/shelves re-arranged in a more logical and tidy manner and therefore space available for dispensing.

Consultation room to be cleared of stock/boxes etc
