The pharmacy does not always store its medicines in an organised and safe manner. Medication with short expiry dates is not routinely marked and medicines requiring disposal are not segregated from in-date medicines. Fridge temperatures are not routinely recorded. The space for storing completed multi-compartment compliance packs is not adequately managed which creates a significant risk of error.

The shelf with all the medication in bottles has been cleared and put in the medication waste bins. All old dossettes (multi-compartment compliance packs) have been put in the medication waste bins. Going forward all medication from old dossettes go straight in the medicines waste bin.

Fridge temp. After informing all the staff verbally and on the WhatsApp I have found that in the last month a few days have been missed again. I have now downloaded a fridge temp log which I will attach to both fridges and assign certain staff to complete it daily. I will inform all the pharmacists to make sure it is up to date on their day.

Dosettes-I have two proposals-
1) All dossettes which are prescription checked will be bagged as normal. Any dossettes which are only accuracy checked will be bagged in a clear bag, until they are prescription checked.
2) All dossettes whether accuracy or prescription checked will be bagged, the prescription checked dossette will have the prescription attached to the bag.
