The pharmacy does not effectively assess and manage the risks with all of its activities. It does not have adequate governance arrangements to ensure team members complete activities in the safest and most effective way. This includes the way they store medicines, keep records and the changes the team makes following mistakes.

1. Branch to tidy shelves and make sure that different strengths of the same drug are stacked separately and clearly segregated
2. Dispensing errors to be reported to Superintendent Office promptly when identified
3. Monthly Patient Safety report to be completed and team patient safety huddle completed to discuss outcomes and actions
4. Shelves to be sorted and tidied and photos sent to Superintendent office and Inspector
5. CD cabinet to be organised and stock reduced to reduce chance of errors
6. Weekly balance checks to be implemented for CD’s
7. SOP’s to be read and signed by all staff
8. Field support team to provide more intensive support during the period that the branch has no manager
