The Responsible Pharmacist record is not properly maintained so it may be difficult to establish who was responsible at any given time.

Action has been taken by informing all staff & locums about signing in via Proscript. Plan is to have a monthly audit to ensure pharmacists are signing in and out.

Members of the pharmacy team are not all suitably trained for the tasks that they carry out which means they may not always complete tasks in an effective manner.

I have ensure staff are enrolled and have checked they have received formal training. Part time member of staff has been put through with buttercups. Delivery driver had also been put through the delivery driver course with buttercups
