Pharmacy team members do not effectively record or analyse their mistakes.

We have created a near miss log which also has a section for a monthly review. All staff will be asked to record any near miss which was not identified on the PMR.
Our new PMR also has a built in near miss log which populates all the wrong barcode scans undertaken and by who. We will run this report every month alongside the paper near miss log and the pharmacist will review the near miss log and put in place next steps and review these with the rest of the team.

Any complaints regarding dispensing errors that come into the pharmacy must be handled according to the SOP attached. The superintendent pharmacist must be informed and a copy of the Dispensing error record should be provided to him to ensure procedures are followed and any actions arising are put into effect.
Any complaints about dispensing errors must initially be dealt with by the pharmacist on duty at the time the complaint is made and a Pharmacy Incident Form (attached) to be completed with next steps.
This is to allow for the fact that the customer may decide to report the error to NHS England or GPHC at a later date and need to keep a log of the initial complaint and steps taken with regards to it.
Where the patient has taken incorrect medication, the patients GP needs to be contacted as soon as possible or if not possible then 111 to inform them and agree next steps
