The pharmacy has made some improvements since the last inspection. It has implemented some new standard operating procedures and risk assessments. But these do not cover all aspects of the service. For example, supporting the team to understand the importance of people's dispensing history when making clinical assessments, to be able to effectively record and audit clinical interventions, and to manage people's registration with the pharmacy's website to confirm their identity and manage their consent to share information with their regular NHS prescribers. So the pharmacy still cannot fully demonstrate that it comprehensively identifies and manages all the risks associated with the service it provides.

Revised consultation form which will require mandatory GP details.
Patient photos will be required as well as 1 form of ID for age verification.
Prescriber to provide signed confirmation to adhere to established prescribing pathway and treatment justification.
Paper copy of the Prescriber’s qualification to be acquired.
Pharmacist to highlight if returning patient has more than 1 consultation form and check for changes/ discrepancies against history. Follow up question to be asked if necessary.
All interventions to be logged and ‘tagged’ in pharmacy communications program and made available to audit.
Communication between pharmacy and prescriber to also be logged in communications program.
SOP’s to be established for all of the above.
