The pharmacy uses unreliable record keeping systems which means records are not always accessible.

IT team to create a solution where the prescription log records are independent of the Patient Medication Record (PMR) system. This should mean that in case maintenance is required on the PMR, there should be no impact on the prescription log as it will be stored in an independent service on its own. No IT updates that would impede accessibility to records will be permitted during normal working hours until the independent prescription log record has been created.

The pharmacy's website which hosts the prescribing service, includes a medicine for weight loss which is not licenced for this indication. This is against regulatory guidance on the promotion of prescription only medicines outside the terms of their licence. And the website included reference to discounts which promoted specific prescription only medicines.

1) References to Ozempic and Rybelsus for weight loss on the treatment page to be removed and will only be visible once a consultation has been completed as per the MHRA Blue Guide and our communication with the MHRA. The web designer has already created a scope of work and work has begun to implement as soon as possible.
2) The Terms and Conditions will explicitly mention that prescription only medicines (POMs) are not included in any price matching. Also, additional wording will be introduced to the existing price matching content on all pages of the website to explicitly show that POMs are not included as part of the offer. Wording added will be “This excludes Prescription medication” or other wording to a similar effect.

The pharmacy relied on information provided by people in an online questionnaire when prescribing medicines for weight loss. And it did not verify the information provided, such as weight and height. This meant people would be able to obtain medicines that were unsuitable for them if they provided inaccurate information.

1) Update questionnaire to create a unique and comprehensive consultation journey to capture comprehensive information about patient health, history, current and past medication, their lifestyle and so on. This information that can be reused during consultation, compared and verified during multiple touch-points with the patient and their data throughout their weight loss journey. The pharmacy will confirm body weight and height during follow up consultations to re-verify the information and ask the patient the length of time they have been on the same or other weight loss treatments and how much weight they have lost during that period.
2) Body photos will be used to identify vulnerable patients and photo-ID will be used to verify body photos.
3) A suitably trained healthcare professional will use telephone/video consultations to follow up with weight loss patients.
4) The pharmacy will carry out further audits to assess whether information provided in consultation forms varies to the follow up consultations and body photos in September.
5) Checking Summary Care Record (SCR) as part of the weight loss service has already been implemented. SCR checks for weight loss is mandatory at every supply. SCR icons in the patients PMR to be implemented to show if SCR has been checked for a patient. Until this has been implemented notes will be recorded in the PMR.
