Khan Pharmacy (1112225) - Improvement action plan

Pharmacy team members do not record any near miss errors and dispensing incidents. And they cannot evidence any changes they make to reduce the risk of similar errors happening. The pharmacy has a written procedure, but the team members do not follow it to reduce the risk of errors and learn from them.

The Superintendent Pharmacist has put in place an excel spreadsheet that they review. This provides a tool for entering the data and helps with the analysis of the data. On the day of the inspection staff members were unable to locate this document as the Superintendent Pharmacist kept it in a folder. There had never been a need for team members to access the document as the pharmacists notified the Superintendent Pharmacist of near misses to review and do the necessary interventions.

Moving forward all the team members will contribute to the near miss log (not just the pharmacists). And the near miss log has been added to a shared folder on the dispensary computer. All team members have been informed of this.
