The pharmacy does not effectively manage all its confidential information to help protect it from unauthorised access. Pharmacy team members are aware of their responsibilities to help protect people's confidentiality. But they do not always act if there is a risk of information security being compromised.

All staff including the regular locum pharmacists to read and Sign SOP’s to ensure they have read and understood them.
We have introduced a new SOP - preventing errors and recording Near misses
To remind the Pharmacists of their responsibilities for following the SOP
The SOP includes review of error made possible reason and actions taken to avoid in future, providing an opportunity for all team members to learn and improve.

Recording of dispensing errors can be done on the pharmacy PMR system Proscript
The RP Tahira on the day may not have been aware but training to be provided to all staff for this.

The Pharmacy does have a CD returns register kept in the dispensary – The staff on the day were not able to locate it – we have now moved it to be kept with the CD register and all staff have been informed of its location.

Following the inspection and advice given to staff on the day all totes and boxes were removed from the retail area and are now stored in the back of the dispensary.

Nothing is to be kept in an area accessible to the public that could contain sensitive information.

The pharmacy doesn’t adequately protect all its medicines from unauthorised access. And the team does not identify and manage the risks associated with this.

The above includes any stock medication boxes – on the day it seems a staff member had moved an excess stock box of the sildenafil into the retail floor near the totes to make space to do their daily prayer and forgot to put it back – we have taken the decision not to keep any excess medication stock in the consulting room instead to use the excess stock drawers in the dispensary

The door separating the retail area and access to behind the counter to remain closed at all times – all staff to ensure they close it behind them and it is closed after any visitors or delivery drivers deliver

I am not sure why team members on the day said they provide leaflets only when new medication is prescribed – all the leaflets are provided to all patients each dispensing episode – some confusion on why this was not relayed to the inspector on the day

Mixed batches – all staff including pharmacists have been informed to ensure they do not mix batches for the shelf stock – they must keep stock medication in original boxes so batch number and expiry dates are easily identifiable

The pharmacy does not have appropriate equipment to accurately measure liquid medicines. And it does not keep the measures it does have in a suitable condition to use.

The measures were immediately ordered and replaced following the inspection to British standard CE marked -
All staff have been briefed on how to use and maintenance these
