The pharmacy doesn’t keep a signed record, as its written procedures requires its team to do, when medicines are delivered. So, it can’t show it has delivered the right medicine to the right person.

Implement a paper-based signature log for all deliveries. There will be a date, signature and name of the person receiving the medication.
Train staff on proper documentation procedures.
SOPs will be reviewed and improved where required.

The pharmacy’s compliance pack service isn’t adequately managed. And compliance packs are routinely supplied without packaging leaflets. So, people don’t have all of the information they need to take their medicines safely. The pharmacy doesn’t always supply valproates in their original manufacturer’s packaging. It sometimes supplies valproate-containing medicines in a compliance pack without completing an appropriate risk assessment, and warning stickers aren’t added to the compliance pack’s packaging. And its team doesn't provide a patient card or a patient information leaflet when a valproate is supplied in a compliance pack.

Review compliance pack procedures.
All patients will be offered PILs at the point of pick up or delivery. If the patient/ patient’s representative does not require/ want the PILs, this will be recorded.
PILs will be provided for any new medication.
We will supply valproate-containing medicines in original packaging with appropriate labelling where appropriate. If after risk assessment patient needs sodium valproate (or other risk medications) in a blister, PILs will be provided and warning stickers will be attached to each pack.
Blister backing sheet will have a link to access PILs and information about patient’s condition.
Train staff on risk assessments and mandatory patient information distribution.
SOPs will be reviewed and improved where required.
