So, people don’t have all of the information they need to take their medicines safely. The pharmacy doesn’t always supply valproates in their original manufacturer’s packaging. It sometimes supplies valproate-containing medicines in a compliance pack without completing an appropriate risk assessment, and warning stickers aren’t added to the compliance pack’s packaging. And its team doesn't provide a patient card or a patient information leaflet when a valproate is supplied in a compliance pack. |
