The pharmacy does not always make adequate records when people receive the wrong medicine. Pharmacy team members do not demonstrate appropriate learning following the mistakes they make during the dispensing process. And pharmacists do not act in a timely manner to follow the correct reporting processes when they find a discrepancy in the controlled drug register

1.2.1 Area Manager visit and investigation into GPhC findings.

1.2.2 Six colleagues to complete a re-briefing on the critical SOP’s

1.2.3 Regular weekly and monthly Patient safety reviews to be completed by Pharmacy team, Line Manager and Area Manager – Includes Near Miss and Incident reviews

1.2.4 Re-brief to all Team Members and Locum Pharmacists the importance of communicating Patient incidents – re introduce the Communications diary.

1.2.5 Email all locums booked to work or have worked in the last 6 months.

1.2.6 Re-brief the CD Reporting Requirements and the requirement to investigate all discrepancies identified within 24 hours. Unresolved discrepancies to be reported to the CD AO.
