The pharmacy team members discuss dispensing mistakes when they are identified. But they don’t record these in the near miss book. And there is not a robust process for regularly reviewing these dispensing mistakes. So it may be harder for them to identify any patterns or trends and take effective action to help prevent any recurrences.

A patient safety champion has been enrolled. Staff are to record any dispensing errors on near miss log.
Any dispensing incidents are recorded within PMR.
Patient safety reports will be produced monthly
The Pharmacy Manager and Superintendent Pharmacist will review these quarterly or sooner If need be.

Although most medicines are stored in appropriate conditions some medicines are not all stored safely and securely.

Procedures have been introduced to make sure that medicines are always stored appropriately and safely.

While the pharmacy has suitable equipment and facilities for most of the services that it provides. It does not have the facilities to store all its medicines safely.

Stock control and new dispensing processes will ensure that the pharmacy has the facilities to store all its medicines safely.
