The pharmacy is unable to show that it has robust governance processes to ensure that medicines supplied to people are safe to use. It has not considered all the risks around storing medicines outside of their original containers before using them to assemble compliance packs. Or the risks in storing mixed batches in single stock containers which do not contain all the information required to be able to date check and respond to alerts effectively.

All containers that have deblistered medicines from more than one batch to be disposed in the DOOP bin, immediately.

To suspend the use of all deblistered medication for dispensing until further notice.

To contact the manufacturers of each product via email and/or phone call requesting any data that they may have available about the suitability of the products to be stored in containers once they have been deblistered. Information requested should include:
1. Sensitivity to light
2. How long can they be stored deblistered in containers and
3. If other precautions need to be taken.

Once the above information has been received then dispensing of these products to resume if suitable, otherwise, they are to be disposed in the DOOP bin.

Once we have received information about the suitability of dispensing deblistered products from containers then to write an SOP if appropriate or to stop this practice altogether if inappropriate.
