The pharmacy doesn’t adequately identify and manage the risks associated with its vaccination service. It doesn’t have written procedures to make sure this service is delivered consistently and safely. And it hasn’t done enough to make this service safer when something has gone wrong.

Action Plan to improve Vaccination Service:
•The vaccination service will be risk-assessed. A risk assessment template will be saved on the pharmacy’s computer and will be completed monthly.
•Patient group directions (PGDs) will be put into place for the vaccination service.
•Standard operating procedures (SOPs) will be developed for the service.
•All relevant staff will read, sign, and implement these SOPs.
•Regular audits will be performed to ensure the safety of the service.
•One staff member will be designated to support risk assessments and audits to maintain the safety of the service on a regular basis.

The pharmacy cannot show that it stores all its medicines which it needs to keep in a refrigerator at the right temperature.

Action Plan to Maintain Fridge Temperature (to keep the fridge temperature between 2°C and 8°C):
•Order and install Labcold Pharmacy fridges.
•Order and install thermometers to keep track of temperature of each fridge.
•Pharmacy team will record and monitor the temperature range of each fridge regularly.
