The pharmacy team had not read the latest pharmacy procedures (SOPs). Several of the pharmacy processes had changed considerably when the new computer system was installed, so the team may not have been following the correct procedures.

The Pharmacy team will be required to read and sign all PMR related SOP’s with immediate effect.

All staff members have access to the training videos for the new PMR on their individual login. Staff members can sign themselves off as being competent for each of these topics. All topics will be required to be signed off by each member of staff.

The impact of the new PMR system has been inadequately monitored and reviewed. This has meant that issues with staff training and effect that the system has had on the pharmacy's processes have not been identified and addressed promptly.

Cornwells will ask the company expert to carry out a second audit and ensure training of the nursing home module for all branches which provide nursing home services.

Cornwells will continue to listen to staff issues with the new PMR and react accordingly and promptly as we already shown.

The safety and efficiency of the pharmacy's services have been negatively impacted by the new PMR system. The quality of the service provided to the care home is hindered by the implementation of the new PMR system and the working relationship between the care home, the pharmacy and the surgery.

The company expert will provide training again for the branches who provide services to nursing homes.

A meeting will be set up between the surgery, care home and pharmacy to discuss plans going forward. The Superintendent Pharmacist and HR manager will be in attendance. The HR manager is a previous care home manager (of many years) and a CQC inspector of nursing homes, so she will be able to offer insight to the issues faced. If required, we will be able to utilise this member of staff going forward. A clear plan of expectations needs to be agreed by all parties.
