Members of the team do not consistently record things that go wrong so they can learn from them and so they may miss some opportunities to improve.

The SI has reminded all dispensary team members to record all near misses using the QR code, which is easily accessible from the dispensary. The responsible pharmacist will hold monthly meetings to ensure all near misses/DIRFs are discussed, and learning opportunities are shared with wider team.

The pharmacy does not always manage confidential information properly or dispose of it securely. This could result in people’s personal information being disclosed.

We shall provide secure cable ties for all members of the team. We have reminded all staff to secure any confidential waste.

The pharmacy does not carry out risk assessments for all the medicines it supplies at a distance. This means it cannot always demonstrate that the supplies are made safely. And further proof is not always requested when medicines are sent to alternative addresses which could mean people are supplied with medicines without appropriate identity verification.

Clinical lead is working to ensure there are comprehensive risk assessment for all conditions available on the PD portal including for the OTC / P medication supplied over the website. Additionally, the lead is currently reviewing the Asthma and STD risk assessments.

Where alternative address is requested. IT teams will add additional features to help processing team to identify alternative address and mitigate any potential identity issues.

The pharmacy does not always store its medicines securely and in accordance with legislation.

The pharmacy will review our current layout to ensure completed prescriptions are stored away from patients and appropriate space is available in the dispensary for assembled prescriptions. The consultation room will remain free from patient identifiable data as part of the pharmacy cleaning process. A joiner will be appointed to secure shelves from unauthorised personal.
