A new medicine risk assessment tool is being created to review suitability of any new medications before application to the website. This assessment will comprise of product and prescribing risks and how it is mitigated and managed. Further Audits or Review will be required following initial assessment. |

This includes:- verifying the information provided by the person completing the online questionnaire, sharing all relevant information about the prescription with the patient's regular doctor and ensuring effective monitoring is in place. |
Medications requiring additional monitoring such as levothyroxine, diabetic medications, Testogel, inhalers will require up to date checks, supporting documentations will be required. A 3,6 or 12 month assessment tool will be applied for medications such as levothyroxine and diabetes, the system will identify that up to date evidence/documentation will be required. For Testogel, this tool will identify evidence requirements every 6 months. Consultant documents and blood work would require Patient Name and Date of issue to be clearly stated. Saxenda 12-week Assessment tool will be applied, this tool works with algorithms rather than relying on manual monitoring which identifies the initial weight/BMI. After 12 weeks the system will automatically flag that the new order request requires a weight review, the BMI will be clearly stated and require assessment to see if 5% weight loss has been achieved as per criteria. Where a patient has given consent to share treatment information with their GP, our application will print and save the information sent and received to and from their patients GP. This information will be securely saved to their order profile/notes tab.
