The risks associated with providing pharmacy services are not appropriately identified and managed. There is evidence that a mistake has happened because of this as described under Principle 4 and the pharmacy has not handled this incident appropriately. It has weak governance procedures in place. And it has no risk assessments or audits completed to verify that the services that it provides at a distance are safe.

Responsible pharmacist to read and sign all the SOP’s. -

Complete near miss sheets fully with no gaps and carry out a monthly review.

Re-visit Complaints procedure and ensure aware of the step by step process of dealing with complaints. -

Provide risk assessment and risk review audit from 2019/2020 PQS and current PQS.

- Review and revise Handling Incidents SOP.

- Ensure smartcards are only used by the registered user and any personal log-in details are not shared in any form.

- NPA insurance renewed, expires 28/02/21

Ensure the use of unlicensed medicine’s conformity certificates by all dispensers, include patient label, prescriber label and medicine label, signed by RP.

- SOP for online sales of medicines

- Important updates to be made to the online pharmacy website including: Complaints process and how to provide feedback.
• Method by which patient’s personal and private information is protected.
• To inform user’s that it is run by HI Weldricks LTD and provide their contact details, which can be used if there are any problems.

The pharmacy does not have robust procedures in place to raise concerns when medicines or medical devices are not fit for purpose. There is evidence that people's safety has been compromised by failing to act or raise concerns about medicines that have been recalled. The pharmacy is not receiving all of the drug alerts issued by the Medicines and Healthcare products Regulatory Agency. And it cannot demonstrate that it has actioned the drug alerts appropriately.

Delivery pharmacy subscribed to MHRA drug alerts, check daily. Can be accessed by all members of staff.

- MHRA alerts will be printed, acknowledged and signed by staff and the RP. Also, an MHRA drug alerts folder created in the email.

- Care homes to be provided with relevant drug alerts and patient information leaflets.

- Apply for waste license.
- List of high risk medication printed and clearly visible to all members of staff and where relevant notes made on PMR for additional counselling.
