There is insufficient recording, reflection and learning from mistakes to prevent them from happening again.

Near misses to be recorded on the company near miss sheets.
Any dispensing errors to be recorded on the intranet.
Monthly patient safety report to be completed to reflect on near misses and errors and discussed with team.

The pharmacy does not always have enough staff to manage its workload safely.

Review rota and ensure adequate staff cover at all times.
Groundwork for this has already been completed and extra staff drafted in on days the branch is short.
Staff member to return from maternity leave in January.
Recruitment ongoing for 20 hour position.

Team members in training do not receive dedicated learning time. And, so are inadequately supported.

Not all parts of the pharmacy look professional. Areas are untidy which increase the likelihood of errors. And, the pharmacy is doing some activities for which they have inadequate space.

Clear glass panels in the consultation room door mean that people’s confidentiality cannot be maintained.

To be raised with facilities department.
In meantime, cover to be placed over clear glass to obscure view into the room.

The services are not all effectively managed to make sure that they are delivered safely. The lack of training of some team members means that sometimes the pharmacist is not aware of issues that may need her intervention.

Support from area coordinator and Management Support Pharmacist will mean that staff are better trained.

Ensure storage of medicines is appropriate and within the legal requirements.
Review areas for storage of waste medicines and DOOP bins.
Carry out an exercise to reduce stock holding in fridge.
