The pharmacy team members do not identify and manage some risks.

The pharmacy team will hold meetings on a regular basis where they will assess and discuss near miss & dispensing errors logs and to raise risk concerns or any other general issues.

The pharmacy team do not routinely assess the safety and quality of the services it provides. And, the standard operating procedures are overdue a review.

SOPs to be reviewed by pharmacist. All staff to read and sign relevant SOPs

The pharmacy does not always have enough staff to manage its workload safely. The staffing rota means that sometimes the pharmacist is working on his own.

Staff rota arranged to fill any gaps in cover for pharmacy. One member of staff will work from 11am to 3pm to cover gap between 2:30 to 3pm.

Not all areas of the pharmacy present a professional pharmacy image.

Repairs and decoration to consultation room & floor in dispensary. (In progress)

Not all the pharmacy services are effectively managed to make sure that they are provided safely.

Folders now used for methadone patients and all prepared methadone is labelled.
Wallets are used for compliance aid patients with documents to record all medication changes.
Ensure drug alerts folder is recorded up to date.
