Alongside our process of recording dispensing errors and near misses, we will now carry out a root cause analysis of any errors and near misses and highlight patterns and reoccurrences which will be discussed with the pharmacy team, and summary findings displayed on the staff notice board, This will be done once a month. The CD register will now be kept up to date in a timely manner with CD’s being entered in on the same day and entered out when collected or delivered on the same day. Balance checks are now done weekly. The RP notice is displayed where it is clear to the public to see. Date checking will be continued to be done regularly but now a record and date checking matrix will be put in place. Private prescriptions will now include the prescribers details. When emergency supplies are made at the request of a patient, the reason will now be recorded. |

All controlled drugs are now kept in the main pharmacy building. We have applied for an extension to the premises, to increase our space and to make the storage facility into a ‘bricks and mortar’ building and make it part of the pharmacy. If planning is approved we will also be installing a dispensing ‘robot’ to automate our storage and dispensing process, which should facilitate, date checking, efficiency and decrease dispensing errors and near misses. |

A bin for the disposal of hazardous waste is now available. All medicines will now be kept in their original manufacture’s packaging No prescription only medicines will be stored in the consultation room. Audit trails will now be kept to show MHRA alerts are being actioned appropriately. All packaged medicines including MCAa that have a CD included will be stored in a CD cabinet. (we might need more time for this, instruct a builder etc) |
