There is a lack of governance arrangements to identify and manage the risks associated with the pharmacy's services. Formal procedures are lacking or inadequate, including SOPs, Information Governance procedures and safeguarding procedures

Review SOP’s in order to simplify existing SOP’s and getting rid of unnecessary SOP’s where appropriate.
Complete Safeguarding Training.
Review Responsible Pharmacist SOP’s and fill in and sign where appropriate.
Audit CD balances on a regular basis and keep records.
Review and fill in IG folder where appropriate.
Get details of Local Safeguarding Contacts and file.
Patient returned CD’s should be recorded in the returned CD register upon receipt and then signed out when destroyed.

Pharmacy records are not adequately maintained. This includes records of controlled drugs and of private prescriptions.

Get up to date with the private prescription register and CD register and update on a daily basis.

Work benches are cluttered and untidy. This reduces the efficiency of the dispensing operation and may increase the risk of error.

Give the pharmacy a good tidying up, getting rid of a lot of unnecessary paperwork and clutter. Get filing cabinet to keep important paperwork together and for easy access. This will enable the freeing up space for dispensing.
Generally organising the pharmacy to reduce clutter.

Patient returned medicines are not always stored or managed safely

Once the pharmacy is more organised and has more space, the patient returned medicine boxes can be stored in the back dispensary to ensure safety. Destruction of returned medicines should be done daily (weekly at the very most) if possible, to ensure the easy management and storage of waste medication to maximise space.
