1.1

The pharmacy does not identify and manage all the risks to its services. It does not recognise the risk of not acting on drug recalls. And it does not have up-to-date procedures for all its services that team members follow to minimise risk.

The Pharmacy will undertake a review of
procedures in each of the services which it
undertakes. This will ensures these
procedures are up to date
The review will involve the appropriate
staff/team members for each of the procedures and ensure that each of them is
aware of and follows the SOPs pertaining to
the work being carried out. This shall be done
with a view to minimising risk within the
operation of the Pharmacy.

The current pharmacy team members have little direction on how to record and analyse errors. There is little evidence of sharing any learning and making changes to prevent recurrence.

The Pharmacy will implement a robust system
for recording errors. All staff involved in
dispensing will be instructed in how to record
such incidents.
Further to this the system will be regularly
reviewed and analysed with any appropriate
learning shared with staff.

The pharmacy does not have robust arrangements for dealing with medicine recalls. And it has no evidence to provide assurance that the team has acted on recent drug recalls. It has not learnt from the previous inspection.

The Pharmacy will review and improve the
procedures in place for dealing with drug
recalls. This will involve the response to
notifications received and ensuring
appropriate record keeping is in place. In
addition staff will be appropriately trained to
implement and maintain the procedures in
order to minimise risk.
