The pharmacy team members have not signed the standard operating procedures (SOPs) and are not following all of them consistently. This increases the risk of tasks not being completed properly and errors happening.

All staff have been instructed to review all SOPs in particular those relevant to their area of work. Once done these were to be signed off. This was initiated before Pharmacist holiday and was to be completed by return.
In addition pharmacy manager to ensure that these SOPs were to be followed consistently going forward.

The pharmacy doesn’t have a robust process for actioning drug recalls. The recording of the receipt and implementation of medicine recalls is insufficient. And the requirements of the Valproate Pregnancy Prevention Programme are not being fully met increasing the risk of people not having the information they need to take their medicines safely, .

Manager to start making sure all Drug Recall Notifications are printed off when emails arrive and retained in specific folder. Ensure system in place making relevant staff aware of these notifications and how they should be actioned where necessary.
All of above included in specific SOP.

Full review of those patients previously prescribed Valproate. Where appropriate for woman of child bearing age liaised with GP to ensure that conditions of a Pregnancy Prevention Programme are met. Also making sure that patients have been appropriately advised and have the information they need.

Ensure procedures in place to meet requirements of VPPP in future including patient information/cards and SOP for appropriate dispensing and checking.
