Improved analysis of near misses and incidents to take place, robust actions in place to address risk and discussion with the team monthly. Include review of previous actions to ensure desired outcome.

Briefing to each team member monthly on the Professional Standard and any actions required.
Re-brief LASAs and use of laminates to all team members and pharmacists to ensure being implemented.

The pharmacy team do not follow company procedures.

Re-brief the team on:
1. The importance and use of PIFs as per SOP and continued use of coloured laminated cards.
2. On the importance of counselling patients on valproate and the use of the warning cards.
3. The correct process for updating the Medisure patient record with changes.

Re train any team members on the relevant SOPs identified by the pharmacist as not followed correctly and pharmacist to sign off team members as competent.

The pharmacy team is behind with their workload which indicates that something is going wrong with staffing levels and working rotas.

DDS workload is now on track.

Workload in store and rotas to be reviewed along with staffing levels.
Review implementation of model day to ensure that all necessary tasks are being completed in a timely manner.

There is evidence that some people may not be getting the advice that they should about their medicines.

Re-brief the team on:

The staff cannot demonstrate that they act appropriatley to make sure that people only get medicines and devices that are safe.

Folder for alerts set up in the dispensary, team briefed on actions to be taken for alerts.
All alerts for 2019 to be reviewed.

Older alerts to be archived appropriately.
