The pharmacy is not adequately identifying and managing the risks with some of its services. The pharmacy doesn't regularly complete some processes it should. Such as regularly checking its fridge temperatures and the expiry dates of its medicines. And so, there is a risk that the medicines people receive are not fit for purpose.

1. Fridge temperature monitoring to be completed daily in line with LP SOPs. All colleagues to review the relevant SOP and re-sign record of competence.
2. Full date check of the dispensary stock holding and then ongoing weekly checks in line with the LP date checking matrix. Date checks to be fully documented and out of date stock to be removed in line with LP shrinkage policy.
3. Full re-launch of Near Miss processes and Safer Care. All colleagues to receive refresher training regarding expected processes and standards.
4. All colleagues to review latest SOP updates and sign relevant Records of Competency. SOP folder to be updated with the new sections.
5. All CD registers to include full heading detail on all pages as per legal requirements.
6. Full clean/de-clutter of the dispensing areas and back areas.
7. Review of bags on shelf and implementation of LP P2P processes for effective ongoing management.
8. Full review of offsite dispensing processes in line with expected protocols. Guidance and implementation for any elements that are found lacking.

The pharmacy does not always have enough staff to manage its workload. And the team members work under pressure to make sure people get their medicines when they need them. This can increase the risks of mistakes. And the team members don't always complete other less urgent tasks. And so, the pharmacy gets behind with its workload.

1. Review of current checking/dispensing position (including CDS/MDS) and support allocated in order to clear any back log.
2. Review of Right People, Right Place, Right Time to ensure colleague rota and skill mix is in line with the current workload. Changes made as appropriate.
3. Current vacancies for 39 hours tHCP and PM to be advertised. All new colleagues to receive Best Welcome induction at an alternative local store prior to beginning work at Brunswick Street.
4. All colleagues to receive allocated weekly training time in line with the retail operations plan, to include Mylearn activities.
