1.1 | The pharmacy's standard operating procedures (SOPs) have not been recently reviewed to ensure they cover current practice in the pharmacy. Not all team members have read the SOPs and so may not be working in the most safe and efficient way. |
Review and update all Standard Operating Procedures (SOPs) to ensure they reflect current practices and regulatory requirements. The updated SOPs will be distributed to all team members including new members, and each staff member has been required to read and sign off on them to confirm understanding. A training session will be conducted to ensure all staff are familiar with the revised procedures. Going forward, the pharmacy has implemented a system for regular SOP reviews and staff acknowledgment to ensure ongoing compliance. |

2.2 | Some pharmacy team members have not been enrolled on or completed an accredited training course for the roles they undertake. This means they may not have the skills or knowledge required to safely carry out their roles. |
Identify the team members who are not currently enrolled on accredited training courses and enroll them onto appropriate programs relevant to their roles i.e. Dispensing/Counter medicine accredited training. This will ensure their knowledge and skill are safe and appropriate for their role in the pharmacy. |

4.3 | The pharmacy does not always store its medicines appropriately. It cannot demonstrate that all medicines requiring refrigeration are stored at the right temperature. And it does not have a robust process for checking the expiry dates of its medicines to ensure expired medicines are always separated from in-date stock. This increases the chances that medicines are supplied to people that are not suitable for use. |
The pharmacy will review and update its procedures for medicine storage. All 3 fridges will be installed with new thermometers. A new system will be implemented to monitor and record accurate fridge temperatures daily, ensuring medicines requiring refrigeration are kept in the correct range within maximum and minimum temperatures. An expiry date checking rota will be introduced to ensure regular checks are carried out and expired stock is promptly removed and segregated from in-date medicines. The pharmacy will be divided into sections to allow a smoother date checking process. These steps aim to improve the safety and reliability of medicines supplied to patients. |
