There is evidence to show that on some occasions the pharmacy's standard operating procedures are not being followed, which may increase the risk of errors. |
The newly-reviewed SOPs had not been printed out at time of the visit. These have now been printed and are being read and signed by all members of staff. Team meeting on 16/10/2024 has highlighted concerns that SOPs may not be being adhered to and importance of this has been clearly communicated. |

4.2 | The pharmacy team routinely delivers some medicines by posting them through people's letterboxes or leaving them in safe places. This practice is not in accordance with the delivery standard operating procedure and it may compromise confidentiality and increase the risk of errors. |
The superintendent pharmacist will be reviewing the SOP to ensure this practice is on a patient-only basis. A process will be put in place that allows posting or leaving of medication in a safe place only as a last resort authorised by the pharmacist with consent from the patient to be recorded in the PMR. This would then also need to be reviewed annually. |
