Records of controlled drugs are not well-maintained

All prescriptions for CD’s which need to be
entered in the CD Register will be marked CDR
during the dispensing process. After collection
all CDR prescriptions will be placed in a
specific file for write up at the end of the day.
Records of CD’s in the register which have not
been used for >1yr to be removed and
archived,- already done. Stock check of all
CD’s ,- already done by pharmacist and
counter signed and checked by dispensing
assistant Lucy Williams.
Lucy Williams will check entry balances on a
four weekly basis. A log of entered CD Classes
used within the four weeks period will be kept
at the front of the CD Register.
CD Register will be checked for missing
headings and rectified.
Patient returned drugs will be more
stringently scrutinised for any CD’s.

The pharmacy team do not make records of patient safety incidents and there is no evidence of learning from mistakes

The SOP “Error monitoring” will be reviewed.
“Dispensing error recording” forms will be
displayed in the dispensary. Any filled forms
will be collated on a weekly basis (Thursdays)
by dispensing assistant Julie Lee and entered
in the “Monthly dispensing error log” and
discussed with all members of the team. Completed forms will be placed in a new
specific file for error recording.
