Team members do not always follow the pharmacy's written procedures creating risk of errors. And they do not adequately manage all risks when carrying out dispensing activities.

All SOPs will be brought up to date and uploaded to PHARMSMART software. Each member of staff to receive an account log in and to read and mark all SOPs as read.
The superintendent pharmacist met with accuracy checkers to bring the accuracy check SOP up to date and new accuracy checking stamps have been ordered. These will be used to stamp prescriptions to allow pharmacists to mark that a clinical check has been performed before passing to the accuracy checker.
All staff now aware that they must sign the ‘disp by’ box on labels when dispensing prescription.
Accuracy checking stamp will be used on all MDS boxes and signed by the dispenser, accuracy checker and clinical checker.
Superintendent pharmacist met with all staff to explain the use of REPORTSMART, part of the PHARMSMART software, and highlighted the importance of its use for near miss recording on a daily basis.
CDRx stock check reminder feature is to be used going forward. This automatically generates a prompts for all CD registers every 4 weeks to make sure that BOTH a stock check and a date check is performed.
