Pharmacy records do not meet statutory and regulatory requirements. Responsible pharmacist records are incomplete and private prescription records are not up to date.

All records to be made digitally using
Pharmsmart app. Training to be provided to all
staff on how to use app.
Locum guide to be produced to provide
information to locum pharmacists on how to
use app and any other information that may
be required in the course of their shift eg
where to find PGDs etc.
New SOPs are currently being produced and
random compliance checks are to be
introduced re SOPs and records.

The pharmacy cannot provide any evidence to show how it learns from things that go wrong. And it cannot give examples of any action being taken to prevent errors being repeated.

Near miss and dispensing errors to be logged
via pharmsmart.
Weekly reviews to be documented and all
recommended changes to be logged and
documented when completed.

Medicines, including controlled drugs, are stored in unsecured areas of the pharmacy and not adequately protected from unauthorised access. Stock medicines being stored untidily, and the lack of expiry date checks, increases the risk of error.

Some medicines are stored in a food fridge which is unhygenic. And the lack of temperature monitoring means the pharmacy cannot provide assurance that they remain fit for purpose.

All medicines to be stored in appropriate
areas including controlled drugs being in CD
cupboard at all times.
All stock to be stored tidily, date checking to
be logged via Pharmsmart.
Random compliance checks to be introduced
and documented.
No food to be stored in Food fridge at any
time. To be checked as part of compliance
check.

The pharmacy does not have effective systems in place to prevent prescriptions being supplied when the pharmacist is not present.

Sign to be produced to inform public of times
when prescription supply is not possible.
All staff to be reminded when prescription/ P
med supply is not possible. Document to be
produced to state that staff have received
training and signed by staff members.
