Not all team members do the right training for their role. And team members do not always receive support to help them complete their training in a timely manner. Team members are not fully aware about the additional guidance about valproate-containing medicines.

All colleagues to be enrolled onto the appropriate training course.

Colleagues are to have 30minutes training time per week documented in their working rota.


LloydsPharmacy employed pharmacist to train all colleagues on Valproate and use of Purple Pack

All colleagues to re-read SOP HRM05 – Supply of Valproate and complete a new Record of Competence. This is to be assessed and signed off by a LloydsPharmacy employed Pharmacist.

The pharmacy team does not use robust systems to prevent people receiving date-expired medicines.

All colleagues to be trained on dispensary date checking processes as per company policy, including the use of short date stickers.

Full dispensary / OTC date checking to be completed, including full clean / tidy to be completed.


Store team to complete weekly date checking tasks as per company schedule.

The pharmacy team cannot demonstrate that it acts appropriately in response to drug alerts and recalls.

All colleagues to be trained on how to access the company Hub portal and complete outstanding drug recalls / alerts.

Rota to be drawn up to ensure that the Hub is accessed once per day as a minimum.

RM to monitor the Hub activity and completion of recalls and alerts each week.
