1.1

The pharmacy does not follow documented processes for all activities such as failed medicines deliveries and the management of controlled drugs.

The staff should be following the sop,s and have been instructed to comply.
Staff have been informed they are allowed to to deal with CD,s as per sop guidelines.
Failed medicine deliveries -The driver is following guidelines and reports any issues to pharmacist on duty.
The driver will have read and signed sops relevant to his position by 13th December 2019.

2.1

The pharmacy does not have enough team members to safely deliver its services and undertake all necessary tasks.

The pharmacy has adequate staff numbers in relation to the level of dispensing it achieves. There has not been any significant increase in dispensing and staffing has been increased over last 6 months despite this.
The managers are responsible for staff cover and all work together using their professional opinion and the work loads of what staffing is required in any one shop to ensure this happens accordingly. If any issues around workload then they contact superintendent. Managers reminded to ask for advice or support if not happy with cover at any time. We trust the judgement of managers and have had no issues in the process. Staffing levels are reviewed by superintendent. We also have our relief managers working as double pharmacist cover in Montrose at numerous times over the months which again is an added extra provision.
CCTV footage is also monitored to view activity.
Pharmacist /staff have been reminded to arrange cover for holidays and sickness as per above and contact superintendent if any issues. 27/11/2019 and Ongoing

4.2

The pharmacy does not manage all instalment dispensing safely including multi-compartment compliance packs and serial prescriptions.

The staff have been reminded on the importance of managing work to ensure dosset boxes are completed in time prior to them going out.
It has also been suggested that dosset boxes could be managed on a Saturday when dispensary is quieter and there are now dispensers working.
Information leaflets are now attached to the first instalment of the dosset cycle. 28/11/2019

Owings – Staff have been reminded that this is part of their job and must be complied with. Relief managers and staff have been reminded to action any balances in a timely manner. 28/11/2019

Instalment prescriptions – We now have in place staff/Pharmacist makeup instalments and place in designated area. On any given day the instalments due for collection are taken from designated area onto the dispensary working bench. At end of day/week staff/pharmacist can clearly see what has not been collected and action accordingly.

4.3

The pharmacy does not store all medicines appropriately with date expired medicines and prescriptions available for supply. This includes a medicine that was identified date expired three years previously.

Date checking is to be done on a Saturday when dispensary is quieter. 28/11/2019

Out of date CD prescription – Pharmacists have been reminded to check all CD prescriptions are in date prior to dispensing and action if any issues. 27th November 2019

We also do a stock take every 6 months and items out of date or near to expiry date are highlighted at that point also.
The CD that should have been destroyed has been highlighted to the CD team and arrangements made to deal with destruction. 27/11/2019
