As explained in the report feedback the open and honest culture in the pharmacy means that staff are learning constantly. We are now recording errors and will review them monthly along with the end of month procedure. We will also record near misses. |

All instalment tablets are now attached together by instalment date rather than drug. This means that counter staff are not self selecting the medication. Our instalments have been tidied and all old patient/tablets removed. This will be an on going process to ensure we stay on top of it. Purple forms have been filled in and all staff trained to fill in the purple form with the initial date of collection if not known. |
