The pharmacy does not keep accurate records. The responsible pharmacist record is not completed correctly and controlled drugs registers are not properly maintained.

The importance of recording errors for future
referencing will be emphasized. SOP’s will be
reviewed and where necessary updated to
more accurately reflect best practice.
Running balances for CD registers are kept,
however audits of these are not done as
frequently as should be. The audits for the
methadone oral solutions will be done on a weekly basis. Discrepancies to be investigated immediately and resolved. SOP’s to be amended accordingly. All responsible pharmacist’s have been informed of the need to sign in at the start of their shift, and to sign out at the end. A notice has also been placed in the dispensary reminding them to do this.
