Standard not met | Reason | Action being taken by the Pharmacy | By when | Notification By Pharmacy Improvements Made |
---|---|---|---|---|
1.1 | The pharmacy doesn’t have any standard operating procedures (SOPs) available for the services it provides, including safeguarding the welfare of vulnerable people. So, the team members can’t refer to them to ensure they provide services safely and effectively. The pharmacy has some local students working on occasions. So, it may be difficult for them to understand how to work in a consistent way. |
The SOPs are back in the pharmacy, updated and paperless in the computer for reference. They include safeguarding. They have also been emailed to the relevant staff and I am awaiting an email back to confirm they have been read and will be followed. |

1.2 | The pharmacy doesn’t have a documented process to record near misses or dispensing errors. It doesn’t have any forms available for the team members to complete. And it doesn’t have any completed forms for the team to review. There is limited evidence of any actions taken following mistakes. |
The LOGs are back in the pharmacy. Any entry with regards to near misses and dispensing errors are reviewed and if necessary the SOP is updated |

4.3 | The pharmacy doesn’t have robust processes in place to manage its medicines. It doesn’t make records of the checks it makes on expiry dates. And the equipment the pharmacy uses to check fridge temperatures does not work or is difficult to use. So, it can’t be certain its medicines are always fit for purpose. |
The LOGs are back in the pharmacy. We have a log for expiry date check. A new wireless thermometer has been installed. |
