The pharmacy does not adequately identify and manage some of the risks associated with providing its services. It has not considered all the risks with providing the NHS ‘Pharmacy First’ service before starting to provide it. And staff do not always follow the pharmacy’s Standard Operating Procedures (SOPs), which also require review.

The pharmacy has decided to suspend the service till all relevant requirements to provide the NHS ‘Pharmacy First’ service is fulfilled.

SOPS- have been reviewed and signed by pharmacy team members including the superintendent pharmacist.

The pharmacy team members have been advised to sign medication upon dispensing and checking, they have been advised to check the dispensing SOPs and correct procedure.

A near miss book is already in place, pharmacy team members been advised about the importance of recording and learning from near misses.

The pharmacy cannot show that it stores its medicines that require storage between 2 and 8 degrees Celsius correctly.

Fridge log. The superintendent pharmacist has ordered a new thermostat for the fridge. Also, regular temperatures will be taken at opening and closing time daily.
