The pharmacy does not make adequate records when people receive the wrong medicine. And its team members cannot demonstrate learning from these types of events.

Standard operating procedures (SOPs) are being recirculated and a clear flow chart of the necessary steps required if an error occurs is being assembled for the branch.
Weekly learning logs will be undertaken, and weekly error logs are to be sent to Superintendent Pharmacist (SP) to oversee actions requested are being complied with.

The pharmacy does not store all its medicines and equipment as it should. It stores some over-the-counter medicines and equipment in an unhygienic environment.

All medication and equipment will be stored adequately and to required standards. Due to the lack of storage at this premises we intend to move excess equipment to our sister branch a few mins drive away.

Patient returns will be dealt with at the end of each day to prevent build up. They will not stored in unsuitable places.
