Pharmacy team members have not signed all procedures relating to their roles. And there is evidence that they do not complete all tasks in accordance with these procedures. There are occasions where pharmacy team members work outside of their defined roles by undertaking some tasks without the direct supervision of a pharmacist. This obscures the lines of accountability and may increase risk.

Make sure that every member of the pharmacy team has read and signed all the procedures relating to their role within the pharmacy.

Pharmacy team members do not always identify the risks in the way they deliver services such as the supply of medicines in multi-compartmental compliance packs. And they don’t demonstrate how they work in accordance with procedures for this service.

Make sure that every member of staff involved in the supply of medicines in multi-compartmental compliance aids has read and signed the standard operating procedures for this service.

Discuss possible risks involved in operating this service, and ways to reduce them with the pharmacy team.

A list of high risk medications is to be placed on a wall in both dispensaries, so that everyone is aware of them and will take the necessary steps to protect themselves when dispensing. It should also remind the pharmacist on duty of the importance of providing counselling to patients prescribed these medications, especially when they are first prescribed.

The pharmacy does not consider the stability requirements of some of the medicines it supplies in multi-compartmental compliance packs. And it does not always have appropriate arrangements in place for restricting access to medicines requiring safe custody.

The pharmacy's standard operating procedures for the supply of medicines in multi-compartmental compliance aids will be changed so that it considers the stability requirements of the medications they contain if they are made up in advance.

Complete a review of the procedures relating to medicines requiring safe custody to ensure these are in accordance with safe custody requirements at all times.
