The pharmacy does not adequately identify the risks associated with all its services. And it does not take suitable action to properly investigate and resolve problems after it has identified them.

Staff to be retrained to ensure that standardised procedures are followed. Ongoing compliance to be monitored.

Where problems are identified ensure that team members respond by taking corrective action in a timely manner. Ensure that they also conduct a root cause analysis and take further action to prevent the same problems from being repeated.

Ensure all staff understand and follow processes to identify, investigate, resolve and minimise risk.

The pharmacy does not complete all of its records in the way it must. And it does not take sufficient action to investigate issues arising from them.

Review and update all outstanding records. Ensure all essential records are made in a timely manner. And ensure they are complete and up to date.

Retrain staff to ensure they understand and follow processes to identify, investigate, resolve and minimise risk. And to ensure records are completed as required by law.

Review staffing levels to ensure the team can keep up to date with its workload. And to enable them to carry out their tasks and deliver services in a safe and effective way in accordance with standardised procedures.

Review date checking process to ensure a regular routine process is followed in line with SOPs.

Review stock rotation process.

Ensure that when dispensing part packs, remaining items are stored back in their original manufacturer’s containers.
