4.3

The pharmacy does not do enough to ensure that it manages all the medicines it receives properly.

1) Review Standard Operating Procedure (SOP) No. 5 for prescription assembly by all staff.
2) Amend and Reinforce procedure to ensure safety and appropriate management of received medications as per the SOP. Allocated training time to be allocated to ensure all staff will be aware of the requirements.
3) All relevant staff including the Superintendent Pharmacist (SI) and Responsible Pharmacist (RP) to review and duly sign and date the SOP No.5
4) SI/RP to monitor compliance on an ongoing basis and maintain adherence to maintain patient safety.
5) SI to review procedures to ensure that CDs awaiting destruction are managed appropriately.
