• Electronic CD register is a new software that has been introduced into the pharmacy. In order for all staff involved with the dispensing to be competent with using this CDR module extra training will be arranged. If any discrepancies occur, they will be highlighted to the RP. • Staff that are working on Saturdays have been reminded to ensure that the locums log on the RP register. • Handover notes module on the digital reporting software will be introduced into the pharmacy so that information can be relayed to staff the following day. |

The pharmacy does not have appropriate monitoring arrangements to ensure its team members learn from their mistakes. And it does not act to reduce risk by safely storing medicines in an orderly manner. |
• Need to ensure that new team members have read and signed all SOPS. • We need to create a positive environment, where all staff members record any near misses or dispensing errors that occur. The responsible pharmacist will then check that the near misses or dispensing errors have been entered correctly. • To ease the process of recording near miss and dispensing errors we will introduce a digital reporting software. At key points in the dispensary a QR code will be displayed, so if a staff member needs to record an incident it can be done on a mobile device rather than waiting for a terminal to be available. • Errors will be reviewed and any learning from this will be shared with team members to try and minimise risks. |

The pharmacy does not always store its medicines safely and in a way which reduces the risk of a mistake occurring. Its monitoring processes are not effective in removing out-of-date medicines from stock. |
• To improve stock holding and to minimise risks we will remove high risk and cytotoxic medications from the regular shelves and have a designated area for them. • Staff have been informed to regularly put split packs back on the shelves and to use split packs up first where possible in order to reduce stock levels. • Staff are to re-train on how to carry out date checking in the pharmacy. Any short-dated stock will have a red short dated sticker so that it can be easily identified. • We will introduce date checking log on the digital reporting software. This will help us to monitor and remove out of date stock from the dispensary. • Temperature will be monitored in the back store room and camera moved to a suitable place. High risk and medicines that can be abused will not be kept in the back room. |
