We are now using a template from the Npa, it is based on the Gphc guidelines for recording all errors including near misses. We are then transferring the near miss records as well as all information on actual errors to an error log file. Every week we are then carrying out reviews and audits of such instances with all the pharmacy staff. Finally we are implementing any actionable interventions to minimise the errors from such appraisals. |

The pharmacy manager will complete her dispensing course. All staff are encouraged to continue their ongoing professional development through training courses. Jamie Lee and Olivia are now enrolled to attend a training course about children’s dental health. |
