1.6 The pharmacy does not keep all the records it needs to up to date, particularly its responsible pharmacist records. Responsible Pharmacist records – On startup, a prompt now occurs on the PMR system to identify the responsible pharmacist and record it. This prompt occurs each time the PMR is logged in after the previous RP has logged out. This practice has been put in place effective immediately. The RP is logged out at the end of every day.

The Pharmacy is looking to review all SOPs and setting them in order over the next 10 days so that they are easily identifiable under the headings Essential Services, commissioned Advanced Services, Enhanced services, and finally Private services.
Accessibility of SOPs - The SOPs will be accessible from all computer terminals via a cloud-based system. To ensure all staff have read and understood the SOPs, the Pharmacy is producing a master signing sheet that can be signed by all staff members and locum pharmacists.

Locum pharmacists will be provided a locum file to highlight the location of all SOPs and record-keeping requirements. Our induction process will aim to cover all requirements as well. The induction programme (below is an excerpt of the full list) to cover (depending on responsibility of individual) responsibilities:


1. Pharmacy Information Governance requirements (includes e.g. Data Protection & Confidentiality)
2. Responsible Pharmacist regulations
3. Sale of Medicine protocols
4. Essential services - to include Dispensing SOPs, Repeat Dispensing, Waste, Health promotion, Self-care, Signposting, Clinical Governance
5. Controlled Drugs procedures
6. Private Prescription Book
7. Advanced Services provision - NMS service, FLU vaccination service, CPCS service , Covid vaccination service(s), Hypertension case finding service, Smoking Cessation service (SCS)
8. Enhanced Services provision (Locally commissioned services)
9. Locally set up private pharmacy services (PGD services e.g. Travel vaccination, erectile dysfunction etc) 10 working days from receipt of the finalised report Piyush Amin

The pharmacy cannot show that it stores its medicines requiring cold storage at the appropriate temperatures. And there is some evidence that the temperatures have gone out of the appropriate range.

A new active real-time online temperature monitoring system (MOCREO) has been installed. It records fridge temperatures at regular intervals (every few minutes) and stores them on a cloud-based system. These readings are also sent to the pharmacy email as a daily report or can be accessed at any time via the portal/website. There is an audible alert device that alerts members of the pharmacy if any of the fridge temperatures go out of range, so action can be taken immediately. If temperatures go out of range during pharmacy closed hours, a push notification (remote alert) and email are sent to the superintendent pharmacist’s & pharmacy management team’s mobile phones so they may act on this without undue delay. The system also allows for predictive alerts to act on potential out of range temperatures before they occur. The responsible pharmacist will carry out regular audits to ensure the record is kept up to date.

Date checking process - will ensure the process is maintained on a regular basis. A staff rota within the SOPs will be maintained electronically which is accessible to all staff from any computer to ensure it is signed and kept up to date. Products nearing expiry date will be highlighted to alert dispensers and pharmacists to check before dispensing. The responsible pharmacist will carry out regular audits to ensure the record is kept up to date and countersign & date when done.
