The pharmacy doesn’t adequately identify and manage all the risks associated with the services it provides, including the management and storage of its medicines. It doesn't have suitably robust processes. And it doesn't have accurate records to show to help ensure the safety of its processes. |
The intention is to have a pharmacy log book/folder which will feature paper RP logs, visitor logs, near miss logs, a cleaning rota and staff handover notes as currently all are disparate and of limited functionality. This log book will be implemented now in the old pharmacy to allow aspects like the near miss log to satisfy the requirement. To satisfy the requirement all SOPs will be read and signed by all staff immediately, and then revisited in a few months time once the staff move to the new pharmacy. |

The pharmacy doesn’t always store or manage its medicines correctly. And its processes are not adequately robust. This has not improved sufficiently after the previous inspection. |
Date checking is being integrated into the storage of the medicines within the building using Medidrawers which will contain their own date check log. Discussions have been had regarding the unmarked boxes and cut off strips, as this is something that creeps back in periodically. All staff are being put through new training programmes. The storage of medicines at ambient temperatures is changing to be in a drawer based system. Each drawer is intended to have its own date checking matrix for products contained within the shelf face for accessibility and visibility. The fridge which was in storage has now been delivered and installed. This is a LabCold unit with integrated temperature monitoring, battery backup, a battery backup for the temperature logger, an integrated USB transfer port, alarm test function, and is lockable. The outside edge of the fridge has a pocket for manual temperature logs also, and the same localised date check records. |
