Standard operating procedures do not always accurately reflect the way in which team members complete some tasks such as the dispensing workflow. The procedures are not always reviewed when processes are changed so team members may not fully understand the correct ways of working.

All the sop’s will be reviewed and re-updated ensuring the sop’s are accurate in the way the pharmacy is working.

Members of the team do not consistently record things that go wrong so they can learn from them and so they may miss some opportunities to improve.

Near miss logs, incident reporting is all present and available for the stores to record, however staff need constant reminder to ensure errors are logged. The area manager will now be reviewing all logs periodically to ensure the pharmacy is actively logging data in realistic time frames.

Waste medicines are not stored securely from unauthorised access.

The location of the waste medicine has always been allocated to be inside the pharmacy premises area, since the bin can be moved around we have decided to order a wall fix bin which will ensure its location can not be moved.
